Kentucky Permanent Makeup License Requirements

The Kentucky Cabinet of Health and Family Services regulates permanent makeup as tattoo. According to state law, all practitioners must be registered either with the cabinet or with their local health department.
There is an annual $20 fee for tattoo artists registration.
For Northern Kentucky only: In order to obtain your license, you must be able to demonstrate that you have passed a blood-borne pathogens course in order to get your license. The Northern Kentucky Health Department offers these classes. For information on upcoming classes, please call 859.341.4151 between 8 a.m. and 5 p.m., Monday-Friday.
